I’d much rather propose an NHS style health care but beyond that, I think the be all end all is going to have to be some kind of full time support like an inpatient unit even if people want to refuse care. Often in the US “offering medical care” is a poor attempt because it really just means “be here at this time”, “take these meds” etc with out offering rides or transportation or without offering a real solution for those distrustful to take medication the actually really need .
